Survey Templates are utilized when scheduling a survey; they determine survey metrics as well as how a survey will display when administered to participants.

Survey Templates are manually created by navigating to Main Menu > Surveys > Survey Form Designer and clicking +Add Survey Template.

Via the Add Survey Template pop-up, define the details of the new Survey Template.

Enter a name for the Survey Template that is at least 5 characters in length, and select the applicable type of survey from the Survey Type drop-down. Click Save to create the new Survey Template.

The Survey Template Homepage will display and the new Survey Template will be in Draft status. Prior to publishing, how the Survey Template will display to Survey participants can be customized via the Survey Template Designer and Survey metrics can be configured.
