Survey templates are used when scheduling a survey to determine survey metrics and how the survey will display when administered to participants.

Survey Templates are manually created by navigating to Main Menu > Surveys > Survey Form Designer and clicking +Add Survey Template.

Enter a name for the template that is at least 5 characters in length, and select the type of survey (1); once a new template is added, the survey type cannot be changed.

By default, the Survey Template Homepage will automatically display once the template is added (2). The Survey Template Homepage will display, and the new template will be in Draft status. Before publishing, survey metrics can be configured, and the template’s display to survey participants can be customized via the Survey Template Designer.

When disabled, the template will be created, and the manager will reload to display the new template in Draft status.
